Correct. If you didn't know him you wouldn't notice he was thinner than before, but Kagome did so she can see the difference. He's not back to his regular pre-coma weight and muscle shape until the very end of the story.

And Sesshoumaru wasn't built like a bodybuilder to begin with. He was in SHAPE, yes, but also tall and thin at the same time.
